Can translators who are working with documents that are required to be certified please provide samples of the letters they get notarized and attach to the translated document please?

I will be translating an entire family's documents for immigration, (birth certificates, family books, marriage licenses, etc) and they all must be certified translations. I would like to be able to see sample of the certification letters, just to make sure mine are up to snuff.

... but I'm not sure if you are asking us for samples of the types of originals, or samples of the translations.

Letter of Certification May 22, 2013

I'm asking to see samples of the letters you use to certify the translation.

Something along the lines of "I, Fulanita de Tal, certify this fake document to be an accurate and faithful translation of fictitious document belonging to John Doe."

I do my own translations and things, but this will be the first time I've ever needed to get the work notarized, so I'm looking to see what is out there in regards to letters to certify the translation.

Our certifications look something like this (depending on the circumstances):
I, [translator’s name] , hereby certify that the attached XX-page translation from XXX into English is a true, accurate and correct representation of the [name of the document], and that I am a professional language specialist fluent both in XXX and standard North American English, qualified to render such translation.
You will need to add the date and a space for signature, with your full name and ad

You will need to add the date and a space for signature, with your full name and address and professional credentials, like ATA certification. Don’t sign it yet. And then you have to add a line for the notary public, something like “Sworn to and subscribed before me on May 22, 2013 etc. (it’s different in every state, look at Internet if you find a Notary certification for the place where you live). Take this page to the notary and sign before them when they tell you. The notary needs to attach their seal and signature on the same page.
