On the certificate this has been translated to "Mayor" but this is a district/ward in Tokyo, so I would like to use"District Chief of Suginami". Would this be appropriate?

: Personally, I prefer 'ward' for 区, but you're right, it calls itself 'Suginami City'. http://en.wikipedia.org/wiki/Suginami,_Tokyo . Yes to 'mayor' also.
