I agree, though, that the asker's idea of Notice of Change in Tenants (vs. Long) Lessees is the right idea expressed in lay terms.
Interesting that this question has been voted Non-Pro as 1. the Australian term may not been 'on all fours' with the US AmE, CanE or 2. the BrE term of a Landlord's / Landlady's alt. Freeholder's 'Licence to Assign' or 'Consent to Subletting' both of which I - half a century after working in that very room of the City of London law office - have found very few people, apart from Professional Conveyancers and (BrE) Estate Agents / (AmE / CanE) Realtors are familiar with.

Answer: Licence in Australian spelling, license in American spelling. NB The correct spelling for the verb (in Australia) is license, but the noun is always licence.
A tenant can make a written request at any time to assign their tenancy to someone else.

If the Australians wouldn't capitalise "form" neither should we be particularly confused and misled by the fact that a random Swiss person may have done so (in their French-language approximation to the expression ... ).

Obviously the precise wording there is "Change of Tenant/Resident form". There seem to be various permutations. An Australian person might be able to supply the most common form ... but the chances are that the Swiss person saw the words I'm suggesting.

Below is a reference which assumes a change in the place of work brings a reason to change address.
A tenant who chooses to move to another address should provide sufficient notice to the rental authority. He or she may end the tenancy, consider sub-letting or even a home swap arrangement with another tenant.
There's a possibility also that a landlord may decide to end a tenancy for professional reasons and provides notice to the existing tenant.
