Hi everybody, thank you for your help but unfortunately, I need to close the question. The client called the physician whose hardly legible handwriting I was trying to decipher, and it turned out to be that CPE stands for 'complete physical examination', and the words 'with concomitant' should be crossed out, so the the whole phrase should read: Diagnosis: CPE; upper respiratory symptoms including cough.
