Source text - English Definition and Pathogenesis An abscess refers to a collection of purulent material, walled off from the surrounding structures. It is the end result of an inflammatory reaction secondary to a bacterial infection. Most cutaneous abscesses start as a cellulitis over a wide area. Host defenses then cause localisation of the infective process, effectively preventing spread of the infection. When fully formed, the abscess has central bogginess and is fluctuant. ‘Pus’ is a pyogenic abscess consisting of bacteria (both dead and viable), neutrophils, dead host tissue and other inflammatory cells. The contents are invariably acidic due to hypoxic metabolism. The abscess cavity contains many septations within a cavity which segregate the pus into compartments or loculi. This collection of pus is walled off from the surrounding host tissue by dead tissue known as the 'pyogenic membrane‘. This membrane is not only effective in preventing the spread of infection from the abscess to the host, but it may also prevent the entry of antibiotics. Therefore, pyogenic abscesses require drainage of the contents and ideally removal of this membrane for resolution. Presentation Spontaneous rupture of an abscess is not a desirable outcome, as the drainage is usually inadequate and the scar is poor. This manual is for patients and families who have been referred to The Hospital for Sick Children (SickKids) to be assessed for liver transplant. The goal of liver transplantation is to improve the quality of life for children living with end-stage organ failure metabolic liver disease and liver cancer. The transplant team includes many healthcare professionals who want to help every child and family through the transplant process. ABOUT THE SICKKIDS LIVER TRANSPLANT PROGRAM The Liver Transplant Program at SickKids started in 1986. Just over 10 years later, in 1997, SickKids performed its first living related liver transplant. The program expanded again in 1999 to include intestinal (bowel) transplants. To date, surgeons at the hospital have performed more than 400 liver transplants. ABOUT THIS MANUAL This manual gives you general information about transplantation. It also describes the most common practices at SickKids. We will discuss the information in the manual with you at different points during your child’s transplant journey. We have tried to include as much as possible about liver transplants. However, because organ failure is very complex, this manual will not describe everything that may happen. The manual tends to focus on the possible problems related to transplantation, but remember that liver transplantation in children is quite successful. Please talk to us if you do not understand any of the information in this manual or if you have questions that we have not answered. The liver transplant team is a group of health care professionals who will be helping you and your child throughout the transplant process. You will find out a little more about each team member over the next few pages. Transplant nurse The transplant nurse is involved in all areas of your child’s care, from the time your child is assessed to long after transplant. The transplant nurse: • Knows your child’s condition • Teaches you and your child about transplant • Can answer your questions and explain medical information • Co-ordinates your child’s care with other members of the team My transplant nurse is: Transplant nurse practitioner The nurse practitioner (NP) is a registered nurse with extra education and training. This person can prescribe medicines, order tests and do physical exams. The nurse practitioner may attend to all of your child’s medical needs while they are in hospital and after being discharged. My transplant nurse practitioner is: Gastroenterologist The gastroenterologist is a doctor who specializes in diseases of the stomach, intestines and the liver. This person may be the doctor who first tells you that your child might need a transplant. The gastroenterologist will provide the special medical care that your child needs while waiting for a transplant. Your gastroenterologist may also share your child’s care with the transplant doctor if they are not from a transplant hospital. My gastroenterologist is: Transplant doctor The transplant doctor is a hepatologist with special training in caring for transplant patients. A transplant doctor will see your child when your child’s doctor would like an opinion about your child’s need for transplant. Many transplant doctors work together as a team and they will follow your child from the time they are listed for transplant. They will oversee all medical care for your child after the transplant both in the hospital and at home. My primary transplant doctor is: Transplant pharmacists The pharmacists work with the doctors and nurses to adjust your child’s medicines so that your child will get the most out of them. The pharmacists will teach you about your child’s medicines and how to manage them at home. My transplant pharmacists are: In-patient Nursing staff Registered nurses take care of your child during their stay in the hospital. They will also teach you about your child’s liver disease, transplantation, medicines and the procedures that are being done. Transplant surgeon A transplant team surgeon (doctor specially trained to operate) will also see you and your child while your child is being assessed for transplant. The surgeon will perform the liver transplant operation. They will also care for your child in hospital after transplant, and later you may see them in the Liver Transplant Clinic for check-ups. My transplant surgeon is: You will meet many nursing staff during your child’s hospital stay. Social worker The social worker helps you, your child and your family to cope with personal, family and financial issues that may arise from your child’s illness. The social worker provides counseling and therapy. They also help you find financial and practical assistance and can put you in touch with community agencies. My social worker is: Residents and fellows Residents and fellows are doctors who are becoming specialized in an area of medicine. There are two types of residents and fellows: • Surgical residents and fellows are part of the surgical transplant team • Paediatric residents and gastroenterology fellows are part of your child’s medical transplant team of doctors You will meet many residents and fellows during your child’s hospital stay. Physiotherapist The physiotherapist (PT) assesses your child’s physical abilities, including their muscle strength, tone, joint range, posture, balance, and movement. The PT helps to develop a plan of care. They will suggest different treatments to help your child develop their motor (movement) skills as best they can and stay as physically fit as possible before and after transplant. My physiotherapist is: Occupational therapist The occupational therapist (OT) looks at your child’s feeding and developmental skills (such as hand-eye co-ordination).The OT can provide support if your child needs help in these areas while in hospital and, if needed, can connect your family with services in your community for when your child goes home. My occupational therapist is: Dietitian The dietitian helps your child with their diet to make sure they get enough of the nutrients they need before and after transplant. After transplant, the dietitian will assess your child to see if your child needs to follow a special diet. My dietitian is: Transplant information coordinator The transplant information coordinator works closely with the transplant team to coordinate all your child’s appointments and takes care of the day-to-day running of the transplant office. The information coordinator will be your contact for questions about appointments and other general information. My transplant coordinator is: Child life specialist A child life specialist provides healthy, age-appropriate activities for children in hospital. For instance, they help your child to express their fears and worries through play. My child life specialist is: Child psychiatrist A child psychiatrist is an expert in helping children and families with the stress of a life-threatening illness, hospital stays and surgeries. They work together with the social worker at the request of the transplant team or the child and family. My child psychiatrist is: Chaplain The chaplain can offer support to your family during the transplant experience. The chaplain is available to your family regardless of your religion or faith. Therapeutic clown program Therapeutic clowns are available to your child in the hospital. They use play to help your child overcome any fears they may have. About the Liver A FEW FACTS ABOUT YOUR LIVER • The liver is the largest solid organ in the body. • The liver is in the upper right side of your abdomen (belly). • Your liver needs blood to work. • Two vessels bring blood to your liver: - The portal vein brings blood from the stomach, spleen, and intestines to the liver. This blood is rich in nutrients and provides the liver with 75% of its blood supply and 50% of its oxygen supply. - The hepatic artery provides the liver with blood that is rich in oxygen. • Blood leaves your liver through hepatic veins. • Your liver produces bile, a brownish yellow fluid that helps your body digest fats. Bile is carried from your liver to your gall bladder and small bowel (intestine) through a group of tubes called bile ducts. • The gall bladder, which stores bile, is connected to the liver through bile ducts. When your child receives a transplant, the gall bladder is removed from the donor liver. Your child’s gall bladder will also be removed at the time of transplant. Detail of the liver, blood vessels and bile ducts WHAT YOUR LIVER DOES The liver does many important things! It removes waste. The liver removes some waste products that are dangerous to the body. It breaks down drugs. The liver breaks down (metabolizes) and helps get rid of certain medicines from the body. It filters bacteria from the blood. Special cells in the liver act as filters to remove bacteria from the blood and help fight infections. It produces protein. The liver makes proteins. Two examples are: • Albumin – this helps stop fluid from leaking out of blood vessels so there’s no swelling or puffiness • Fibrinogen – this helps blood clot normally It produces bile salts. The liver makes bile, which contains bile salts that help break down and absorb fat from food. The bile drains away from the liver through bile ducts. It helps blood to clot. The liver cells make factors (I, II, V, VII, IX, and X) that people need for normal blood clotting. The liver also makes factors that help to dissolve clots. These factors help to keep a balance between too much and too little blood clotting. It breaks down sugars (carbohydrate). The liver breaks down sugars in our diet so our bodies can use the sugar as energy. It stores vitamins and minerals. The liver stores Vitamins A, D, E, K, and B12 as well as sugar and minerals such as magnesium, iron, and zinc. SIGNS THAT THE LIVER IS NOT WORKING PROPERLY Liver failure happens when your child’s liver is not able to perform its normal jobs. There are lots of signs and symptoms of liver failure. You may see only a few, or in some cases, all of these signs and symptoms. Jaundice When the liver is not working properly, your child’s skin and the whites of their eyes can turn yellow or green. This is called jaundice. It happens when bile cannot leave the liver because the bile ducts are blocked or too small. As a result, bile builds up in the liver and the levels of bilirubin (a bile pigment) in the blood increase. The high level of bilirubin causes the yellow/green colour. Pruritus Pruritus is another name for itching. It happens when bile salts build up in the skin because bile cannot drain out of the liver normally. This itching may be very difficult to control. It may stop your child from sleeping normally and make them more irritable when they’re awake. Pale stools Bile gives the stool (poo) its normal brown colour. Your child may have pale (milky beige) stools if bile does not reach the intestine. Tea-coloured urine Urine should be pale yellow. When urine is tea-coloured, bile is not leaving your liver normally. Ascites Ascites is the build-up of fluid in the belly. It happens when the liver cannot make enough albumin to help keep fluid in the blood vessels. Edema This is another name for swelling. It happens when fluid leaks into other parts of the body, especially the lower legs, feet and ankles. Weight loss Your child may lose weight if the liver cannot break down and absorb the nutrients that the body needs from the diet. Your child’s abdomen (belly) may get bigger because of ascites and push up on the stomach. This may make it hard for your child to eat normal amounts of food. Your child’s feeding may also be affected by itching and sleep loss, which may make them more irritable. All these things lead to poor nutrition and weight loss. Not absorbing enough fat-soluble vitamins Vitamins A, D, E, and K are called fat-soluble vitamins – they are stored in the liver and fatty tissues of the body. Bile helps your child to absorb these vitamins, but if the bile cannot get to the intestine, your child may not absorb enough of them. This can lead to different health problems, depending on the vitamin. • Not enough vitamin A: eye sight problems • Not enough vitamin D: rickets (weak bones) • Not enough vitamin E: problems with nerve growth and red blood cells • Not enough vitamin K: slower blood clotting More bleeding than normal When the liver is not working normally it cannot make the factors that help the blood to clot. Signs include: • Bruising more easily • Bleeding from the gums or nose Blood in stool or vomit If the liver is not working properly, blood that usually goes to the liver through the portal vein can back up (in the circulatory blood flow system) because the pressure is too high. This can cause bulging veins in the esophagus (the tube that carries food from the mouth to the stomach), stomach or intestines. The bulging veins are called varicies. Varicies are a problem because they can bleed, leading to blood in the stool or vomit. Call your doctor right away if you see any blood in your child’s stool or vomit. The bleeding may start to happen more often and in larger amounts. Take your child to the nearest Emergency Department if your child vomits a large amount of blood or passes it from their rectum (bum). Transplant doctors or gastroenterologists can try to control bleeding from blood vessels using sclerotherapy or banding. • With sclerotherapy, a medicine is injected into or around the bleeding vessel. This causes it to clot and stop bleeding. • With banding, a tight rubber band is wrapped around the vein. These procedures are done under general anaesthestic (your child is asleep and feels no pain).They often have to be done several times to get good control of the bleeding vessels. Varices may re-develop in the future. Muscle wasting If your child’s liver has not been working normally for a while, their nutrition will be poor. This can cause the body to break down muscle into protein. We try to stop this from happening by giving your child extra nutrition in different ways, such as: • High-calorie diets • Nasogastric feeds - feeds through a tube that goes in through the nose right down into the stomach • TPN (total parenteral nutrition) - nutrition through the bloodstream Kidney failure When someone has a very unhealthy liver for some time they have end-stage liver disease. Not only does this affect the liver, but it can also prevent the kidneys from working properly. Hepatic encephalopathy The liver helps clear toxins from the body. When the liver is sick, these toxins can build up in the body and brain. This can cause your child to feel sleepy, irritable or confused. In some cases, this may lead to a coma. Your child will be assessed for transplant when your child’s doctor is worried that your child’s liver is not getting better. The goals of the assessment are to: • Decide if a transplant will help your child • Decide if your child needs the transplant soon or later in their life • Discuss if any other treatment options will help your child such as medicines, nutrition or physiotherapy • Give you and your child information about liver transplantation • Give you and your child a chance to meet the transplant team An assessment for transplant usually takes 3 to 5 working days. It usually happens on an outpatient basis (your child will not stay in the hospital overnight). If your family is from out of town, our team can assist you in finding accommodations for your stay in Toronto. If your child needs to stay in the hospital because they are sick, the assessment will be done while they are here. WHO WILL HELP ASSESS YOUR CHILD You and your child will meet the following members of the transplant team during the assessment: • Transplant nurse • Transplant nurse practitioner • Transplant doctor • Transplant surgeon • Dietitian • Social worker • Physiotherapist • Occupational therapist • Child life specialist Other specialists such as a cardiologist (heart doctor), a nephrologist (kidney doctor), an ophthalmologist (eye doctor), a dentist, and others may also see your child, depending on your child’s medical condition. HOW WE ASSESS YOUR CHILD The transplant team will do a number of tests to help decide if a transplant is needed. Tests that are always done: Abdominal (belly) ultrasound This is a scan to look at your child’s organs in more detail. Blood tests Blood tests give us information about your child’s liver and kidneys, what your child is vaccinated against and what infections they have been exposed to. We test all children for HIV (the virus that causes AIDS) during the transplant assessment. ECG ECG is short for electrocardiogram. This studies the electrical activity of the heart, for example how quickly and strongly the heart beats, and the size and position of the main parts of the heart, called chambers. ECHO This is an ultrasound of the heart. It is a scan that checks the size and shape of the heart and if the heart has been damaged in any way. GFR This is a test that checks the kidneys’ ability to filter toxins. X-rays We do a chest x-ray to look at your child’s lungs and other organs, as well as an x-ray of your child’s wrists to check the health of their bones. You can get more information about these tests in Appendix IV, on page 61. Tests that are done only sometimes (depending on your child’s medical condition) • CT scan • MRI • Mesenteric angiography • Upper GI series • Metabolic survey of the hands and wrist • Liver biopsy • EEG (electroencephalogram) – a study of the electrical pulses of the brain • Auditory and visual evoked potentials (measures hearing and eyesight) What happens after the tests The transplant team will review all the test results to decide if and when your child needs a transplant. This can take about two weeks. The transplant nurse will call you to discuss everything once the team has reviewed all the information. In some cases your child may need more tests to help the transplant team make the best decision about whether a transplant is needed. We will explain these tests to you before they are arranged. If the team decides that your child needs a transplant soon, your child will be placed on the Liver Transplant Waiting List. Types of Liver Transplants A person can receive a liver transplant from a: “Living donor” – a parent or other relative, a friend or an anonymous person who donates a piece of their liver. “Deceased donor” – someone who has suffered: - Brain death (doctors have verified that the brain is no longer working) - Cardiac death (doctors have verified that the heart cannot support life) The family of a deceased donor must agree to donate their loved one’s organs. All patients who need a liver transplant are placed on the trans plant waiting list for a deceased donor. Families also have the choice to have a live donor transplant. If a family chooses live donor transplantation, the child is kept on the waiting list for a deceased-donor organ in the meantime. LIVER TRANSPLANT WAITING LIST As its name suggests, the Liver Transplant Waiting List keeps track of patients who are waiting for liver transplants. The list ranks and organizes patients (children and adults) so that the right patient can get a donor liver when it is available. National List The list covers all of Canada and since mid-2012, has been managed by Canadian Blood Services. The only patients that go onto the national waiting list are patients with fulminant liver failure (rapidly progressive liver failure without a prior known liver disease). Canadian Blood Services requires that we get your permission to collect, use and disclose personal health information for the sole purpose of placing a patient on the waiting list. If you agree, our transplant nurse or transplant information coordinator provides Canadian Blood Services with all the information it needs to place a child on the national waiting list. Please talk to your transplant nurse or doctor if you have questions or concerns about sharing personal information. Provincial List This list is maintained by the Trillium Gift of Life organization (TGLN). It is TGLN that notifies SickKids that there may be an organ available for a specific patient. HOW THE LIST IS ORGANIZED Each patient on the transplant waiting list is given a “Score”.
