An analysis of 30 plant events and the results of 32 peer reviews between 2003 and early 2005 indicates significant weaknesses related to operator performance especially the control of core reactivity. This analysis was subsequently used as one input to develop WANO SER 2005-2, 'Weaknesses in Operator Fundamentals', published in September 2005.
Key Observations
1. Half of the events involved problems with the control of core reactivity. Changes to core reactivity often resulted in a plant transient or reactor scram.
2. Insufficient supervision contributed to adverse consequences experienced in 28 events (90%). Peer reviews confirm that supervisors are not monitoring and reinforcing expectations during plant activities as expected by senior plant managers. In some cases, the supervisor was absent from the control room.
3. The lack of procedure adherence occurred during 16 events (53%) and was a primary cause of performance problems during peer reviews. In addition, operators inappropriately used abnormal and emergency operating procedures during simulator training.
4. Plant management did not observe or monitor some infrequently performed plant evolutions such as reactor start-ups to support the operating shift and provide feedback on the implementation of management expectations.
Overall Conclusions
1. Weaknesses in operator performance, especially a lack of sensitivity and appreciation for core reactivity, resulted in significant events. The analysis identifies specific operator performance weaknesses in the following areas:
§ Reactivity control
§ Operator parameter monitoring and plant control
§ Supervision of plant activities in the control room

Kľúčové pozorovania
1. Polovica udalostí zahrňovala problémy s kontrolou reaktivity aktívnej zóny. Zmeny v reaktivite aktívnej zóny mali často za následok nestacionárny stav elektrárne, alebo havarijné odstavenie reaktora.
2. Nedostatočný dohľad prispel k nepriaznivým dôsledkom v 28 udalostiach (90%). Partnerské kontroly potvrdzujú, že dohliadajúci pracovníci nemonitorujú a neposilňujú predpoklady počas činnosti elektrárne tak, ako to očakávajú riadiaci pracovníci elektrárne. 3. Nedostatky v dodržiavaní postupov sa vyskytli počas 16 udalostí (53%) a boli primárnou príčinou pracovných problémov počas partnerských kontrol. Okrem toho, operátori nevhodne používali neobvyklé a núdzové prevádzkové postupy počas výcviku na simulátoroch.
4. Manažment elektrárne si nevšímal, alebo nemonitoroval niektoré zriedkavo uskutočňované zmeny v elektrárni, napríklad spúšťanie reaktora, aby podporil prevádzkovú smenu a poskytol spätnú väzbu na realizáciu očakávaní manažmentu.
Celkové závery
1. Nedostatky v činnosti operátora, najmä nedostatok citlivosti a porozumenia pre reaktivitu aktívnej zóny, vyústili do významných udalostí. Analýza identifikuje nedostatky v špecifickej činnosti operátora v nasledujúcich oblastiach:

1.1 The gas-net system idea
is the generic term for an Elster device family. The evaluation computer of the gas-lab Q1, too, is based on the gas-net device family. All gas-net devices, including future device types, are characterised by uniformity in appearance, operation and parameterisation.
Each Elster device always covers a multitude of measurement and control functionalities.
gas-net devices also provide this functional variety. To keep the operation and parameterisation of the devices well structured and user-friendly, the gas-net series is based on a modular concept. A module corresponds to a special functionality. Each module has its own main display within the device’s menu assistance, and each module has its own group of settings within the parameter data record.
A particular module can be employed in different device types. This yields a modular system that is advantageous to the user as a particular module can always be operated and parameterised in the same way, no matter in which device type it has been installed.
1.2 The measuring principle
The gas-lab Q1 measuring system is a gas quality analyser that performs infrared absorption and thermal conductivity measurements. The primary target variables are the gross calorific value, standard density and CO2 content of the natural gas being measured.
The entire measuring system consists of two components:
1. gas-lab Q1 sensor system
The actual sensor technology is located in an explosion-proof housing, which can be installed in a hazardous environment.
2. gas-net Q1 evaluation computer
The evaluation computer of the Q1 measuring system is located in a non-hazardous environment. The main tasks of the evaluation computer are controlling and monitoring the measuring process, evaluating the sensor measurements, calculating the target variables and supporting the user during calibration. Furthermore, the evaluation computer contains an integrated data logging function, mainly for interval- and event-controlled data logging of analysis data as well as of error listing and logbook. The signal and message processing provides some digital and analogue output possibilities. The evaluation computer also controls the measuring process and calculates the target variables.

1.1 Koncepce systému gas-net
je společný výraz pro rodinu přístrojů Elster. Vyhodnocovací počítač gas-net Q1 je také založen na rodině přístrojů gas-net. Všechny přístroje gas-net, včetně budoucích typů zařízení, jsou charakterizovány jednotností vzhledu, provozu a parametrizace.
Každé zařízení Elster obsahuje množství měřících a řídících funkcí.
Přístroje gas-net poskytují také funkční rozmanitost. Aby se udrželi funkce a parametrizace zařízení dobře strukturované a uživatelsky příjemné, je řada gas-net založena na modulovém principu. Modul zodpovídá jedinečné funkčnosti. Každý modul má svoji hlavní obrazovku v rámci podpory přístroje přes menu a každý modul má svoji vlastní skupinu nastavení v rámci záznamu údajů parametrů.
Konkrétní modul může být použitý v různých typech zařízení. Toto poskytuje modulární systém, který je výhodný pro uživatele, protože konkrétní modul může být vždy provozován a parametrizován stejným způsobem, bez ohledu na to, do kterého typu zařízení byl nainstalován.
1.2 Princip měření
Měřící systém gas-lab Q1 je analyzátor kvality plynu, který uskutečňuje měření infračervené absorpce a tepelné vodivosti. Primární cílové proměnné jsou výhřevnost/spalné teplo, normální hustota a obsah CO2 v měřeném zemním plynu.
Celý měřící systém se skládá ze dvou komponentů:
1. Senzorový systém gas-lab Q1
Současná senzorová technologie je umístěna do skřínky v nevýbušném provedení, která může být nainstalována v rizikovém prostředí.
2. Vyhodnocovací počítač gas-net Q1:
Vyhodnocovací počítač měřícího systému Q1 je umístěn v bezpečném prostředí. Hlavní úlohy vyhodnocovacího počítače jsou řízení a monitorování měřícího procesu, vyhodnocování senzorových měření, výpočet cílových proměnných a podpora uživatele v průběhu kalibrace. Kromě toho, vyhodnocovací počítač obsahuje integrovanou funkci záznamu dat, hlavně pro záznam intervalových dat a dat řízených událostmi z údajů o analýzách a také z výpisu chyb a deníku. Zpracování signálů a správ poskytuje některé digitální a analogové výstupní možnosti. Vyhodnocovací počítač též řídí měřící proces a vypočítává cílové proměnné.

Obsah oxidických vtrúsenin vo vzorkách č. 1 (z povrchovej oblasti bramy) bol veľmi rozdielny. V štyroch prípadoch bol obsah oxidických vtrúsenin vyšší a smerom k vzorke č. 2 klesal, v ďalších prípadoch bol obsah v povrchovej oblasti nižší a smerom k vzorke č. 2 stúpal. V oblasti medzi povrchom a stredom bramy (vzorky č. 2 a 3) bol u väčšiny tavieb zistený rovnaký priebeh, keď obsah oxidických vtrúsenín mal približne vyrovnaný charakter, bez väčších zmien jedným alebo ;druhým smerom. Smerom k stredu bramy (vzorka č. 4) obsah oxidických vtrúsenin až na dva prípady vždy stúpal. Toto rozloženie so zvýšeným obsahom oxidických vtrúsenin v stredovej' oblasti plynule odlievaných brám je všeobecne známe4) a bolo autormi zistené v plynule odlievanej brame z konštrukčnej ocele5).
Překlad - angličtina In most samples there was very low content of sulphide inclusions. These inclusions were exclusively sulphides of manganese MnS, small globular and oval particles. In many places there was found out creation of clusters which is characteristic for the type MnS II. In spite of low content of sulphide inclusions, it was possible to find out from the data in the Tab. II, that in all melts there was the lowest content of these inclusions in samples No. 1, in five from seven studied melts this content rose and achieved its highest value in the sample No. 4, from the centre area of the slab.
Distribution of oxide inclusions was not as steady as in the case of sulphide inclusions. Graphic illustration of distribution of oxide inclusions and of total content of inclusions from the surface to the centre of continuously cast slabs is in the picture No. 3. Content of oxide inclusions in samples No. 1 (from the surface area of the slab) was very different. In four cases the content of oxide inclusions was higher and in the direction to the sample No. 2 decreased, in further cases the content in surface area was lower and in the direction to the sample No. 2 rose. In the area between surface and the centre of the slab (samples No. 2 and 3) there, at most of melts, was found the same course, when the content of oxide inclusions had approximately balanced character, without bigger changes to one or the other direction. In the direction to the centre of the slab (the sample No. 4) the content of oxide inclusions always rose, with exception of two cases. This distribution with increased content of oxide inclusions in the centre area of continuously cast slabs is well-known 4) and was found by authors in the continuously cast slab of structural steel 5).

V roce 2012 provedl VUZ modernizaci zabezpečovacího zařízení, při které došlo zejména k:
-doplnění stávajícího staničního zabezpečovacího zařízení 2. kategorie osazením výhybek elektromotorickými přestavníky,
-zřízení počítacích úseků k znemožnění přestavení výměn výhybek při jejich obsazení kolejovým vozidlem,
- instalaci pomocných stavědel u vybraných výhybek,
- modernizaci sdělovacího a přenosového zařízení dodávkou elektronického zapojovače ovládaného prostřednictvím dotykového panelu s možností připojení digitálních i analogových linek a s možností stupu do mobilní sítě GSM-R,
- dodávce a instalaci indikátoru plochých kol k zajištění dohledu nad technickým stavem a správnou funkcí kol vozů a hnacích vozidel, pohybujících se a zkušebních tratích ZC VUZ Velim.
Dynamický zkušební stav
Součástí ZC VUZ je hala dynamického zkušebního stavu pro provádění pevnostních a únavových zkoušek části vozidel a drážních zařízení, která je blíže popsána v 17. kapitole o akreditované zkušební laboratoři, jejíž je součástí.

The VUZ performed in the year 2012 modernization of security system, which consisted mainly of:
-completion of existing station security system of the 2nd category by equipment of points with electromotive point-operating apparatuses.
-establishment of counting sections in order to disable movement of rail switches during their occupation by railway vehicle.
- installation of auxiliary interlocking at selected turnouts.
- modernization of communication and transmission equipment by delivery of electronic hook switch, controlled through touch panel with possibility of connection of digital and analogue lines as well as possibility to enter to the mobile network GSM-R,
- delivery and installation of indicator of flat wheels to ensure the supervision of technical condition and correct function of wheels of carriages and driving vehicles moving on test tracks of the ZC VUZ Velim.
Dynamic test bench
The part of the ZC VUZ is the hall of the dynamic test bench for execution of strength and fatigue tests of parts of vehicles and railway facilities, which is more closely described in the Chapter 17 on accredited testing laboratory, of which it is a part.
I attended and completed Technical College, field Communications and Electronic Devices. Subsequently I graduated on the Technical University Bratislava, Slovak Republic in the field of study Communications and Electronics. Finally I obtained degree PhD on the Slovak Academy of Science in the field Measurement Engineering.
I was employed for about 20 years in the Slovak Occupational Safety Research Institute as Senior Research and Development Worker. Later I worked four years for the global international Fluor Corporation with headquarters in the USA, on two European construction projects as Safety Manager in English speaking managerial team. During the whole professional career I used English as part of my job.
I am in translation business for 20+ years. Since 2001 I work as full time freelance translator.
With regard to my qualification and previous working experiences I do translations mainly from the following fields: energy / power generation, nuclear power engineering, hydro-electric and thermal power plants, building industry, electrical engineering, automotive/cars&trucs, occupational safety and health protection and environment.
