I thought that 针头 merely means "pinhead", but in the context above it doesn't seem right.

Well, it could be a syringe needle, but that what happened to be in his arm must be a needle head, even when it was a syringe needle head. That was my reasoning and I was so unsure to have my suggestion in question.

I don't think so, because the guy whose arm I'm talking about is walking about complaining that there is a "needle head" attached to his arm that requires a doctor's expertise to detach; if it is a syringe I figure he can just pull it off himself.

agree Frank (Wei-fang) Luo : Yes. Syringe = 针筒, needle = 针头. If it's stuck in his arm, it may be a needle that is fixed in place by tape, to which you would attach a drip or other injection device.
